1. Insurance Billing
As a courtesy to our patients, Oliver Winston Behavioral Urgent Care will file a claim to your primary and secondary insurance plans for services rendered.
- Every patient and/or legal guardian is financially responsible for services provided by Oliver Winston Behavioral Urgent Care.
- It is the patient and/or legal guardian’s responsibility to know their benefit coverage and if services rendered by Oliver Winston are covered under their plan.
- Any services not covered under insurance benefits will be billed to the patient.
- Patients must present a valid insurance care at the time of each visit.
- If Oliver Winston is unable to verify the patient’s insurance, full payment will be due at the time of service, or a payment plan can be created (requiring a completed payment plan form and debit/credit card number on file).
- Co-pays are due at the beginning of each visit and Oliver Winston reserves the right to decline services if payment is not rendered.
2. Deductibles
If patients have a deductible, it is likely that the service rendered will go towards the deductible, and payment in full is expected at the time of service.
3. Self-Pay
Oliver Winston Behavioral Urgent Care offers self-pay options for patients who do not have or do not choose to use their health insurance coverage.
- Payment for services is due before services can be rendered or a payment plan can be created (requiring a completed payment plan form and debit/credit card number on file).
- The self-pay fee schedule is equal to twice the amount of the Medicare fee schedule.
4. Sliding Scale
Sliding Scale payment options are available for patients falling into the low to moderate-income level.
- Patients requesting sliding scale payment options must present the previous year’s tax return or one month of consecutive payroll/bank stubs.

The amount of the sliding scale will be calculated as follows:
For every $1,000 of annual income, the session rate increases by $2; with a minimum of $100.00 per visit.
- Sliding scale payments are due before services can be rendered. It is the patient's responsibility to discuss payment arrangements prior to the appointment.

We do not accept post-dated checks, nor will we hold checks for any length of time.
Returned Checks
Patients are responsible for any bank fees incurred due to returned checks. A bank service fee of $25.00 per check will be charged to the patient.
